    Community Rehabilitation Programs

    Rehabilitation Treatment in the Community (RTC) Team provides intensive community outreach and psychiatric rehabilitation services to adults with severe mental illness, referred by the Department of Mental Health, residing in Essex County North.

CRS works in the community with clients, referred from the Department of Mental Health, who seek assistance with their behavioral health, medical, social, housing, educational and vocational needs.

    Program for Assertive Community Treatment (PACT) Based on a national, Evidence Based Model, PACT provides comprehensive, locally based treatment for adults with severe and persistent mental illness living in the community. The program operates as a self-contained, mobile team that provides counseling, crisis stabilization, psychiatric medication services, substance abuse treatment, vocational rehabilitation, and housing support services. Clients are referred directly from the Department of Mental Health.

    Community Support Program (CSP) Intensive case management services in the community for clients needing additional support. Funded by Medicaid HMOs, this program is a wrap-around service providing additional case coordination support to meet a client’s multiple needs.  It does not replace, but rather augments any other existing service.

    Southeast Asian Community Clinic (SEACC)

SEACC provides culturally appropriate services in the community with clients, referred from the Department of Mental Health, who seek assistance with their behavioral health, medical, social, housing, educational and vocational needs.
    SEACC is also the coordinating agency for the Metro-Boston Asian Collaborative which provides a full range of behavioral health services for Cambodian, Chinese and Vietnamese adults and their families centered in the communities of Boston, East Boston, Revere and Dorchester.

    Conexions provides structured, short-term day and evening treatment to adults recovering from mental illness and alcohol and drug addiction at the East Boston Counseling Center. Conexions also provides services in Khmer and Vietnamese.
